Ōtaka is a member of the prefectural police who appears in Matsuribayashi-hen. He is the mole in the police used by Takano Miyo. He comes to the Okinomiya police station to confirm that Rika if really dead. It is suggested in conversations by Ōishi that he use to be a subordinate. Thus, Ōishi addresses him with "kun" to irritate him. When Ōtaka protests, Ōishi emphasizes the kun. Ōishi confronts and accuses him of risking the lives of all of the inhabitants of Hinamizawa. Goaded by Ōishi, he tries to punch him, only for Ōishi to execute a jūdō throw.
Ōtaka claims that he is a personal friend of a politician Ōishi fears. When Ōishi asks who the identity of the politician, Ōtaka proudly claims he is Sonozaki Saburō with whom he has a personal relationship. At that moment, Akane and Saburō arrive on the pretext that Akane has to pay yet another speeding fine. Ōtaka does not recognize Saburō, nor does Saburō know who Ōtaka is. Ōtaka can only cringe as his empty threat falls apart.
At the end of Miotsukushi-hen, he orders the Banken to execute Takano to prevent the authorities from finding out about "Tokyo". Hanyū however, protects Takano, sacrificing herself in the process. After the incident, Akasaka vows to investigate Ōtaka and the organization he works for.

Ōtaka has a violet/blue tie, glasses, brown eyes and black hair. He arrives with two other policemen who become cowed first by Ōishi, then by Sonozaki Saburō.
